PROGRAM EXPANSION INTO QUEENS COUNTY tarting in February, 2010, the Long Term Home Health Care Program expanded its operating territory and began enrolling patients in Queens County. Since 1986, our program has been providing high quality home health care to individuals all over the Bronx. Now offering the same, top-notch level of care to the residents of Queens, we renew our pledge to create a “nursing home without walls. ” “We are very pleased to have the chance bring the HHH quality of care across the bridge to Queens,” says Joann Monahan, Director of Patient Services for the LTHHCP. “Patients should take comfort in knowing that we will be bringing the same level of care that we are known for in the Bronx with us into these new areas. ” The move will provide eligible residents of Queens County with the wide range of services the LTHHCPcan offer.
